Sorry it's been so long, but I've been doing a lot of recording and cd printing/burning, built my HTPC/Diy Projector, went on vacation, and started my design on an automatic CD loader, so I don't have to sit there and put the CD's in one at a time, it will have a hopper that will hold 100 CDs and do it automatically!
Anyways the cd printer, as I said uses a Cannon with a straight though paper path, which means you can feed the paper though from the back of the machine instead of from the top, this way the paper doesn't bend it just goes straight though from back to front. The Caddy is just made out of poster board, just print on it like you would a CD label and cut out the hole for the cd to fit, than slap some paper on the back so the cd won't fall though, now you can just take this and feed it though with a printable cd in it and print like your printing a label, the extention is only to hold the paper in place while it's going though the printer, you'll see what I mean when you try it. I don't really want to go into more detail because I belive the guy selling the plans/info is not asking an unreasonable price, I was leary when I bought them but it wasn't a lot of money, and no I'm not affiliated with him or I wouldn't have told you this much, but this printer is a no brainer to print cds with, in the plans there's ways to modify other, better, printers which involve taking the printer apart, so Thats how I justify my telling you only this much. Now My automation system thats a different story...
