Due to the lack of standardization, front panel USB's on cases often terminate in indivdidual pin connectors so that any layout may be accomodated.

It's possible that they are wrong.
The most common layout on mothernboards places the four as:
+5v, Data -, Data +, 0v in a line, but with the second port often staggered or reversed relative to the first.
Ensure that the port 1 and 2 lines have not been mixed up.

There may be an additional line for an additional ground, or for overcurrent indication (never found out how that works) - if the extra case line is protective/screen ground, and the motherboard pin is overcurrent indication, do not connect them.

If the case has a block header, it may be incorrect for the motherboard, as reverse and stagger seem to be used randomly - if that's the case, it may be possible to get ONE set correctly, but both may require re-pinning the connector - on some, it is possible to unlatch and prise out the contacts.

Your Mother board should come with a manual and in there is a fully explanation of the location for the usb front port and pin layout, I know this because I have assembled several systems with this exact model, anyway if for some reason the manual didn´t came with your mobo then look for it at asrock web site and read it, I´m not being rude with you is just that the manual is very graphical and things are much easier this way.

6 ports total - now I know on most VIA chipset systems, that woud be seen as 3 controllers, each with it's own Root Hub, 2 ports each - and generally enabled/disabled seperately in BIOS.

This is Sis chipset - and I don't know how they set theirs out.

Are you already running the latest USB driver
http://www.asrock.com/support/Download/dl_k7s8x.htm
Sis do not distribute the USB driver from their site, so the only alternative that might be more recent is Windows Update
