Well, it depents from what you want. Do you want it for gaming, take the best bargian you can get. In general the cards have the same nvdia gf4 chipset and in the tests the diferent brands stay's within a narrow space side by side. However, the leadtek cards are well known here in europe as a verry good brand, they all have a good cooling and are very good quality. The latest incarnations from leadtek 4200 cards have VIVO. VIVO means Video IN and Video Out. Mostly all cards nowaday's have a Video (TV) Out, but the Video In is another story. So if you want put a video signal in from your VCR (Analog video recorder) or camcorder (DV (Digital Video)) to proces your movie's or for capturing then you need Video In, note that the processing of this cards is in software, so it will use your cpu-cycles. If you search in the forums you get to know that video processing with good results is not easy. Want you try it than is a video-in card (such as you mention yourself) a good and cheap solution. I looked into it lately myself, and my choice is the leadtek 4200.

A OEM card is a card with the driver-disc, but without aditional software (mainly a few games). A REFURBISHED card is a repaired card (mainly wath come's back for guaranty). A RETAIL card is the card with aditional software and full guaranty.

The asus motherbord you mention is a good choice, verry good quality and a good overclocker if you are into it. I do not know if the soyo is any better, because i do not have any experience with it ,soyo do have also good bords, but i'll should say asus.

For the casing question, well, you have a fan for your processor, on the grahpicscard, and one for the supply. This is enough, but maby you can think of a big tower, so the hotter air can go up. Also you can use rounded cables or fold the normal flat cables (Cheaper). Also think of openings in the casing to let the cool air in, and let the hot air out, and do not put your PC anywhere near hot places. But you did already known this i'll presume.

firstly bud, do not buy a refurbished card. it's not worth the hassle.

OEM stands for "Original Equipment Manufacturer". All it means is that there is no packaging and no extra software excluded. OEM pricing is usually considerably cheaper because of this.
