Pioneer A05 with memorex, Maxell, Optodisc, Pioneer DVD-RW's

any RW media I use (Optodisc, Pioneer, Maxell, memorex) always turn out to be coasters after I try to format them.

I have used the following software to format with no luck:
Nero 6
Stomp RecordNow Max
Pinnacle Instantwrite
DvdinfoPro

Any suggestions of which software I should use to format a -RW disc and not have it turn into a coaster?

And is there any software to recover these discs to make them usable?
