Assuming that you are using IE, I would say that as long as you use a utility to clean all the cookies off your system after you browse you should be able to accept all 1st party cookies and 3rd party cookies if they have a compact privacy policy, which means absolutely ****, but atleast eliminates most of the less respectable cookies you could get.
In other words, Medium if your comfortable or Medium-High if your parranoid.
