Which version of ffdshow did you install? See the ffdshow configuration form.
Also there must be one or more .grf files in your output directory. Can you open them with GrapStudio or GraphEdit and post a screenshot here.

On all mkv files I try, the audio extraction of ac3 freezes after 100%. The time counter indicates that the entire ac3 file has been extracted, but nothing happens after this point - CCE does not open to begin encoding. I do not have the same problem with avi or mp4 files.

I have got it working again - I tried uninstalling TFM and ffdshow and re-installing but still no luck. Then I remembered that I also had the TFM Audio Tool installed - uninstalled all 3 and only re-installed TFM. This time it is working again with mkv files.
P.S. - I was unable to find "the ffdshow configuration form"; not sure what you meant by that.
