Are you using Windows 9X and/or FAT32 filesystem?

FAT32 can handle filesizes up to 4GB (or 2GB, with a nonstandard clustersize) and there's no other workaround but using NTFS.

Changing FAT32 to NTSC or PAL is bitta difficult... NTFS makes a bit more sense.
The time it worked the output was smaller than 4GB, that's why it worked.
If it "doesn't explain" to you then I'm afraid we are void of further suggestions.
