Only use pre-cut round labels and a proper applicator to centre them.
You often get an applicator with boxed reatail CD-R software - I did in the Nero 5.5 Retail box.
There are several "styles" of labels, with the placing of the two CD labels per sheet being either offset each way (typically PRESSIT) or in-line (often the BOMATECH).
There are also standard labels, and more recently "full face" with a smaller hole in the middle.
The Bomatech layout also includes hub rings, so you could print an additional piece for the inner, and lay that on the plunger before the CD.
